Alpine Health is seeking a part-time Care Partner in Victoria to manage care plans and coordinate services for SAH participants under the Aged Care Quality Standards. This role involves working with participants, informal carers, and families to plan, implement and monitor care aligned with individual goals.

The Care Partner will ensure dignity, risk-aware decision making, and regular monthly touchpoints, with occasional home visits and collaboration with the clinical care team.

Qualifications

• Experience in Health & Community Settings.
• Minimum of a Certificate III in health, nursing or community related field.

Responsabilités

• Care Management and Service Coordination.
• Deliver person-centred care planning that reflects each participant’s needs, goals and budget.
• Maintain regular monthly care management touchpoints with participants.
• Conduct home visits for admission and care planning on an occasional basis.
• Support the care planning process in collaboration with the team and with participant consent.
