Job summary

Alpine Health in Victoria is seeking a Manager - Support at Home to lead care coordination and management across the CHSP & HACC programs. You will supervise the Intake, Care Partners, and Service Coordinators, ensuring targets are met and high-quality, person-centred care is delivered.

The role requires leadership experience in health services, roster management, and a strong focus on continuous improvement and KPI-driven performance.

Qualifications

• Tertiary qualification in Allied Health or Clinical.
• Experience leading health and community services teams.
• Proven leadership and staff supervision abilities.

Responsibilities

• Lead and supervise Intake Officer, Care Partners and Service Coordinators.
• Oversee assessment and service coordination within Home and Community Care Program (HACC).
• Manage staff rosters effectively.
• Drive recruitment across Support at Home, CHSP & HACC as required.
• Lead service improvement initiatives to enhance care quality.
• Foster a collaborative team culture with emotional intelligence.
• Ensure high-quality, person-centred care for CHSP & HACC participants.
• Coordinate care plans with relevant programs and monitor performance data.
• Ensure compliance with regulatory standards and accreditation processes.
