Job summary

Qualifications

Responsibilities

• Lead intake officers, care partners and service coordinators within CHSP & HACC programs.
• Oversee assessment and service coordination to meet targets.
• Manage staff rosters and recruitment for Support at Home/CHSP/HACC.
• Drive quality improvement and ensure regulatory compliance.
• Foster a collaborative, emotionally intelligent team culture.
• Ensure high-quality, person-centred care for participants.
