Job summary

Gippsland Lakes Complete Health in Victoria is seeking a dedicated professional to manage a caseload of Support at Home Participants within a consumer directed care framework. You will ensure each participant and family receives a tailored, responsive service and you will handle all client documentation and the maintenance of individual budgets.

The role requires a Minimum Certificate IV qualification, with a degree or diploma in nursing or a health-related discipline preferred.

Job description

• $80,008 to $86,588 (pro-rata) per annum, plus superannuation
• Salary Package up to $15,900 per year
• 12-month fixed term position, Full time to part time negotiable
• Lakes Entrance or Bairnsdale location

About the role

You will be responsible for managing a case load of Support at Home Participants. This involves working closely with each Participant and their families to ensure they receive an individualised, responsive service under a consumer directed care model. You will also be responsible for all client documentation and the maintenance of individual client budgets

Skills and Experience

We're seeking someone with a Minimum Certificate IV qualification. A degree or diploma in nursing or a health-related discipline preferable.
