Job summary

Gippsland Lakes Complete Health is seeking an experienced care coordinator to manage a caseload of Support at Home Participants in a consumer directed care model. You will work closely with Participants and families to provide individualized services and ensure accurate client budgets and documentation.

The role offers a 12-month fixed term contract with flexible full-time to part-time negotiation, located at Lakes Entrance or Bairnsdale, with competitive wages and salary packaging options.

Qualifications

• Minimum Certificate IV qualification.
• A degree or diploma in nursing or a health-related discipline preferable.

About The Role

You will be responsible for managing a case load of Support at Home Participants. This involves working closely with each Participant and their families to ensure they receive an individualised, responsive service under a consumer directed care model. You will also be responsible for all client documentation and the maintenance of individual client budgets

Skills And Experience

We're seeking someone with a Minimum Certificate IV qualification. A degree or diploma in nursing or a health-related discipline preferable.
