Job summary

Homewell, part of the Kinyara Health group in Australia, is seeking a Support at Home Lead to drive the success of the program, lead a team of seven Care Partners and uphold quality, compliance and client outcomes. You will partner with operational leaders and clinicians to embed new ways of working, develop capability and deliver personalised in-home care that supports clients to live well at home.

You’ll bring senior leadership experience in aged care or community services, with strong change

Qualifications

Experience in aged care, home care or community services.
Knowledge of the Support at Home program or Home Care Package environment.
Experience leading teams through change and growth.
Coaching high-performing teams.
Understanding quality standards, compliance and risk management.

Responsibilities

Lead and coach Care Partners to deliver exceptional client outcomes.
Embed the Support at Home model and maintain program integrity.
Drive accountability, learning and continuous improvement.
Support complex client care planning, risk management and service optimisation.
Oversee budget utilisation, service growth and sustainable program performance.
Build capability across the team in care planning, funding and client conversations.
Ensure high standards of quality, compliance and documentation.
Partner with stakeholders, providers and clinicians to create exceptional client experiences.
Lead teams through change with clarity and confidence.
