The Aged Care Lead will shape service delivery, translate reforms into practice and lead high‑performing teams across Support at Home and CHSP programs, ensuring a consistent, client‑centred experience across Care

Qualifications

• Demonstrated senior leadership experience within aged care, home care, health, disability or community services.
• Relevant qualifications in aged care, health, community services, business, management or a related discipline.
• Experience leading services through periods of reform and organisational change.
• Knowledge of Support at Home, CHSP and contemporary aged care service delivery models.
• Experience leading leaders and multidisciplinary teams.
• Strong understanding of aged care reform, quality standards, care governance and risk management.
• Experience managing operational performance, workforce planning, budgets and service sustainability.
• A proven track record leading change, transformation and continuous improvement initiatives.
• Strong stakeholder engagement, communication and influencing skills.
• Experience using data and insights to drive operational decision‑making.
• A collaborative and authentic leadership style aligned with Care Connect's CARE values.

Responsibilities

• Lead operational performance across Aged Care programs.
• Drive safe, effective and person‑centred service delivery.
• Lead workforce planning, resource management and operational sustainability.
• Monitor and improve service, quality, workforce and financial performance.
• Provide leadership and oversight of governance, risk and compliance activities.
• Support implementation of aged care reforms and new service models.
• Lead continuous improvement and operational transformation initiatives.
• Build capability, accountability and performance across leaders and teams.
• Develop strong internal and external stakeholder relationships.
• Contribute to organisational strategy, growth and service innovation.
