CatholicCare Central Queensland – Rockhampton QLD
Make a difference that matters
At CatholicCare Central Queensland, we believe everyone deserves the opportunity to live well, maintain their independence and remain connected to their community.
As a Care Partner, you’ll play a key role in helping clients achieve their goals by coordinating personalised care and support that reflects their individual needs, choices and preferences.
This is an opportunity to build trusted relationships, make meaningful decisions and deliver services that genuinely improve people’s lives.
You’ll manage a portfolio of clients across our Community Care programs, primarily within the Support at Home program. As a key point of contact for your clients, you’ll work collaboratively with them, their families and service providers to ensure their care is responsive, coordinated and focused on achieving positive outcomes.
You’ll combine relationship building and care coordination with practical responsibilities, including care planning, assessments, budget management and monitoring service delivery.
You’re a person-centred professional who enjoys building relationships, solving problems and helping people achieve their goals. You can balance the needs of your clients with the practical requirements of service coordination, documentation and budget management.
A formal qualification in Case Management or a related field.
We recognise that our people are at the heart of the services we deliver. When you join our team, you’ll have the opportunity to do meaningful work while being supported in your own professional development and wellbeing.
If you’re passionate about empowering people to live independently and want to contribute to a service that puts clients at the centre of everything it does, we’d love to hear from you.
