Job summary

Mercy Community in Rockhampton is seeking an experienced Care Partner to join our Community Aged Care Services team on a permanent full-time basis. The role requires travel within the Capricorn Coast Region and delivers care management to help older people live safely at home.

You will work in a rights-based, person-centred framework, collaborating with participants, their supporters and interdisciplinary teams.

Qualifications

• Diploma-level qualification in Community Services, Aged Care, Case Management, or equivalent.
• Experience in care coordination, case management, or similar roles in aged care, health, or disability services.
• Strong communication, empathy and cultural competence; skilled with care management or CRM systems.

Responsibilities

• Collaborate with participants to develop individualised care plans within required funding timeframes
• Incorporate the participant’s goals, and environmental requirements within the participant care plan and delivery
• Establish and update care plan and funding requirements, ensuring participants understand their available services and financial responsibilities
• Review care notes and service delivery data

The Care Partner is the designated person under Community Care, responsible for delivering care management that enables older people to live safely and independently at home. Working within a rights-based, person-centred framework, the Care Partner collaborates with participants, their supporters, and interdisciplinary teams to coordinate tailored, responsive and inclusive support.

About you

As the successful applicant, you will possess -
