Job summary

Department of Justice, HM Prison Barwon, Geelong region, seeks a Specialised Lead Assessment and Transition Coordinator to coordinate transition services for people in custody and help them prepare for release.

You will work with multidisciplinary teams to plan release and transition services, complete risk and reintegration assessments, support parole applications, coordinate referrals with prison staff and external providers, and maintain accurate records.

Qualifications

• Experience delivering assessments, programs or services in corrections.
• Knowledge of risk assessment tools and methods.
• Ability to work with multidisciplinary teams in a corrections context.
• Clear advisory skills to guide colleagues through training and knowledge sharing.

Responsibilities

• Coordinate the specialised Corrections Victoria Reintegration Pathway for people with complex needs.
• Complete risk and reintegration assessments and support parole applications.
• Work with multidisciplinary teams to plan release and transition services.
• Coordinate referrals with prison staff and external service providers.
• Maintain accurate records, data and reports on services and outcomes.
