Job summary

The Department of Justice and Community Safety VIC – Geelong invites applications for an assessment and transition coordinator. You will help people in custody prepare for life after prison, assess risks, support parole applications and coordinate transition services.

You will coordinate risk and needs assessments, parole processes and pre-release services, work with prisoners, staff and service providers, and maintain clear records to support programs and parole applications.

Qualifications

• Experience in, or understanding of, working in a correctional environment.
• Understanding of transition and reintegration issues for people in custody.
• Ability to use risk assessment tools with individual prisoners.
• Ability to work with people who may be hard to work with.
• Computer skills using Excel to manage data.

Responsibilities

• Coordinate risk and needs assessments, parole applications and pre-release services.
• Support case management reviews and help track each person's progress.
• Coordinate programs, referrals and services that support people before and after release.
• Work with prisoners, prison staff and service providers to support transition plans.
• Maintain clear records, data and schedules to support programs and parole applications.
