Job summary

Department Of Justice in Victoria is seeking an Assessment and Transition Co-ordinator to support people through their prison journey, assess needs, plan release and coordinate services within a secure setting. You will work with a skilled team to develop transition plans, coordinate pre-release services, and maintain accurate records to support parole applications and program delivery.

This ongoing, full-time role offers a salary of $81,496 to $98,955 per year plus super, and a strong

Qualifications

Knowledge of transition and reintegration needs of people leaving prison
Skills in using risk or welfare assessment tools and making sound referrals
Communication skills to work with diverse groups, especially Aboriginal and Torres Strait Islander communities
Problem-solving skills and ability to work with people who may find support difficult

Responsibilities

Support the Corrections Victoria Reintegration Pathway and assess rehabilitation and reintegration needs
Develop transition plans and parole applications, with clear plans for release
Coordinate pre-release services and work with service providers to support people leaving prison
Work with people and teams to build practical plans for rehabilitation and community return
Maintain accurate records and data to support service planning, parole applications and program delivery
