Job summary

State of Victoria is seeking a Specialised Lead Assessment Transition Coordinator to coordinate transition services for people in custody. You will support complex needs, prepare for release, and connect with community services to ensure safer transitions.

The role involves coordinating the Reintegration Pathway, completing risk assessments, and working with multidisciplinary teams to plan release and transition services, with a focus on accurate records and service outcomes.

Qualifications

• Experience delivering assessments, programs or services in corrections is highly desirable.
• Strong knowledge of transition pathways and risk assessment tools.
• Excellent advisory and coaching capabilities to support colleagues.

Responsibilities

• Coordinate transition services for people in custody.
• Assist with complex needs release planning and reintegration.
• Complete risk and reintegration assessments and support parole applications.
• Collaborate with multidisciplinary teams to plan release and transition services.
• Coordinate referrals with prison staff and external service providers.
• Maintain accurate records, data and reports on services and outcomes.
