Job summary

State of Victoria is seeking an Assessment and Transition Co-ordinator to support people through their prison journey, assess needs and coordinate release plans with service providers.

You will work with Corrections Victoria Reintegration Pathway, develop transition plans for parole applications, and maintain records to support planning and program delivery. Aboriginal and Torres Strait Islander communities are encouraged to apply.

Qualifications

• No mandatory qualification is listed.
• A tertiary qualification in a suitable field is highly desirable.

Responsibilities

• support the Corrections Victoria Reintegration Pathway and assess rehabilitation and reintegration needs
• develop transition plans and parole applications, with clear plans for release
• coordinate pre-release services and work with service providers to support people leaving prison
• work with people and teams to build practical plans for rehabilitation and community return
• maintain accurate records and data to support service planning, parole applications and program delivery.
