Job summary

Coastwide Community Care is seeking a dedicated SIL Support Worker for on-site work in Lake Haven, NSW. The role involves assisting residents with daily living tasks, including personal care, meals, and household duties, while supporting independence.

You will implement individual support plans, promote choice, and help with medication prompts and health appointments. Excellent communication with clients, families, and team members is essential.

Qualifications

• Demonstrated experience as a Support Worker or in a similar disability, mental health, or community care role.
• Strong interpersonal skills and communication skills to build trust, provide person-centered support, and work within multidisciplinary teams.
• Knowledge of safeguarding, duty of care, and compliance with relevant legislation, policies, and risk management procedures.
• Ability to provide basic technical support related to client care (e.g., using electronic care systems, incident reporting tools, and communication devices).
• Relevant qualification in disability support, community services, aged care, or a related field (or working toward such qualifications).
• Current First Aid and CPR certification, NDIS Worker Screening Check, and valid driver’s license.
• Capacity to work shifts, including evenings, weekends, and public holidays, and to perform manual handling tasks safely.
• Respectful, inclusive approach to people of all backgrounds, cultures, identities, and abilities.

Responsibilities

• Support people living in shared or independent accommodation with daily living tasks, including personal care and meal preparation.
• Assist with household chores and community participation as part of individual support plans.
• Implement and promote choice and independence for clients while adhering to safeguarding standards.
• Maintain accurate documentation and incident reports; follow quality and risk management processes.
• Communicate effectively with clients, families and colleagues; participate in team meetings and training.
