Job summary

Sirv in Horsham, Victoria, is seeking a dedicated Team Leader to oversee our Supported Independent Living (SIL) home and ensure high-quality care for participants with complex support needs.

You will lead daily operations, develop and implement tailored support plans, manage rosters, and mentor Support Workers. You will ensure accurate reporting to NDIS/DHHS and collaborate with families and external partners to achieve positive outcomes.

Qualifications

• Experience supporting individuals with disabilities, particularly with complex needs.
• NDIS Worker Screening Check is required.
• Certificate IV in Aged Care/Disability/Community Services plus First Aid/CPR.
• Comfortable working in an environment with cats.

Responsibilities

• Oversee the daily operations of SIL homes.
• Develop and implement tailored support plans and risk assessments.
• Ensure accurate restrictive practices reporting to authorities within required timeframes (e.g., NDIS, DHHS).
• Collaborate with families, teams, and external stakeholders to achieve positive outcomes.
• Guide and support Support Workers to deliver care per plans.
• Lead by example with hands-on, person-centered care.
