Job summary

Sirv in Horsham, Victoria, is seeking a dedicated Team Leader to oversee our Supported Independent Living (SIL) home and ensure high-quality care for participants with complex support needs.

You will lead daily operations, develop and implement tailored support plans, manage rosters, and mentor Support Workers. You will ensure accurate reporting to NDIS/DHHS and collaborate with families and external partners to achieve positive outcomes.

Qualifications

• Experience supporting individuals with disabilities, particularly with complex needs.
• NDIS Worker Screening Check is required.
• Certificate IV in Aged Care/Disability/Community Services plus First Aid/CPR.
• Comfortable working in an environment with cats.

Responsibilities

• Oversee the daily operations of SIL homes.

About The Role

• Oversee the daily operations of our Supported Independent Living homes.
• Develop and implement support plans and risk assessments tailored to participants' individual needs and goals.
• Ensure restrictive practices reporting is completed accurately and submitted to relevant authorities (e.g., NDIS, DHHS) within required timeframes.
• Work collaboratively with families, teams, and external stakeholders to achieve positive outcomes for participants with highly complex support needs.
• Guide and support Support Workers, ensuring they provide care in line with support plans.
• Lead by example—providing hands‑on support to participants and promoting a strong, person‑centered care approach.

About You

• Experience in supporting individuals with disabilities, particularly those with complex needs.
• A Certificate IV in Aged Care, Disability, or Community Services (or equivalent), along with a current First Aid/CPR certificate.
• Proven leadership skills in managing and supervising teams in SIL homes.
• Training & experience in Manual Handling.
• Experience working with individuals with Intellectual Disability, Epilepsy, Schizophrenia (highly regarded).
• Experience in managing rosters.
• A valid NDIS Worker Screening Check.
• Happy to work in an environment with cats.
