Job summary

Claro Disability Services is seeking a Senior Support Worker to lead and provide hands-on support across two Supported Independent Living homes in Victoria. You will partner with families, medical professionals, and other carers to ensure high-quality, person-centered services.

You will oversee mobility assistance, daily tasks, and clinical plans while promoting autonomy and meaningful participation for clients.

Qualifications

• Experience in complex care, disability, and health nuances.
• Person-centered approach and strong communication are essential.
• Reliable, professional with good note-taking and teamwork.
• Flexible and adaptable to shifting needs and shifts.
• Optional but nice: manual handling, epilepsy management, or disability-support training.

Responsibilities

• Lead person-centered planning with clients, families, and advocates.
• Assist with mobility, transfers, positioning, daily tasks, and devices per care plan.
• Monitor health changes, follow clinical plans, and liaise with care team.
• Provide social time and community access aligned with client interests.
• Support decision-making, respect consent, and promote independence.
• Respect and incorporate client backgrounds, beliefs, and culture.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision‑making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being

Roster (worked across both sites):

Week 1

• Tuesday 0700 - 14.30
• Thursday 0700 - 1430
• Friday 0700 - 1000
• Saturday 0700 - 1600

Week 2

• Tuesdays 0700 - 1430
• Thursday 1000 - 1900
• Sunday 0700 - 1600
