Claro Disability Services in Melbourne is seeking compassionate care workers to support clients with complex needs. You will contribute to person-centered plans, assist in daily living activities, and collaborate with families and medical teams to ensure well-being and safety.

The role offers flexible shifts and opportunities to build meaningful relationships with clients while delivering high-quality, person-centered care.

Qualifications

• Candidate is person-centered, warm, patient and genuinely caring.
• Interest or experience in complex care, disability, and health nuances.

• Reliable and professional with good note taking and teamwork.

Responsibilities

• Person-centered planning with client and family/advocates to understand goals and routines.
• Assist with complex physical care including mobility, transfers, positioning, daily tasks and devices as per plan.
• Monitor health changes, follow clinical plans and liaise with the care team as needed.
• Provide social time and community access aligned with clients' interests.
• Support autonomy, advocacy and informed decision-making, respecting consent.
• Respect cultural backgrounds and values in all interactions.
• Perform personal care, meal prep, swallowing management, cooking, cleaning and documentation.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
• Personal care, Meal prep, Following all swallowing management plan, Cooking, Cleaning, Documentation
