BlueCare, part of UnitingCare Queensland, is seeking a Senior Personal Carer to support clients in Rockhampton and surrounding areas. The role involves initial intake, care plan development, and delivering client-centred in-home care, including transport, shopping, and social support.

You will work across CRM & CMS systems, with travel to client homes and community activities. Essential qualifications include Certificate III Aged Care and a valid Australian driver’s licence; a roadworthy vehicle

Qualifikationen

• Certificate III Aged Care or equivalent qualification (Essential)
• Current Australian Driver’s Licence (Green Provisional minimum) (Essential)
• Vehicle available for work (Essential)
• Experience with smartphones, tablets, laptops, etc.

Aufgaben

• Conduct initial client intake and screening assessments
• Develop and review individualised care plans with client-centred approach
• Provide care based on clients’ needs as per Care Plan
• Maintain client information in CRM & CMS with up-to-date notes
• Engage in ongoing care discussions, planning and reviews with care team
• Provide in-home support including daily activities and transport
• Support with community-based recreational and social activities
• Assist with shopping and appointments as required
