Job summary

BlueCare is seeking a Senior Personal Carer in Rockhampton to deliver client-centred care across community and home settings. You will conduct intake and assessments, develop and review care plans, and support clients with daily activities and transport as part of a trusted care team.

Essential qualifications include a Certificate III in Aged Care or equivalent and a valid driver’s licence; experience with mobile devices and CRM systems is valued.

Qualifications

• Certificate III in Aged Care or equivalent required.
• Current Australian Driver's Licence required.
• Experience using mobile devices and basic computing tools preferred.

Responsibilities

• Conduct initial client intake and screening to identify needs.
• Develop and review client-centred care plans.
• Provide care based on individual Care Plans during home visits.
• Manage client information in CRM & CMS, keeping case notes up to date.
• Support clients with daily activities, transport, shopping and social activities.
• Collaborate with care team to assess progress and adjust goals.
