Job summary

BlueCare, part of the UnitingCare Family in Australia, seeks a Senior Personal Carer to deliver client‑centred care in Rockhampton and surrounding areas. You will conduct initial assessments, develop care plans, and support with daily living activities in clients’ homes, while maintaining CRM records and collaborating with the care team.

Qualifications

• Certificate III Aged Care or equivalent – essential.
• Current Australian Driver’s Licence – essential.
• Vehicle for work – roadworthy and insured – essential.
• Experience with technology (smartphones, tablets, laptops, etc.).
• Willingness to satisfy probity checks.

Responsibilities

• Conduct initial client intake and screening assessments.
• Develop and review individualized care plans with a client‑centred approach.
• Manage client information in CRM & CMS; keep notes up to date.
• Engage in ongoing care discussions and goal reviews with the team.
• Travel to clients' homes for daily activities and support.
