Job summary

BlueCare is seeking a Senior Personal Carer to support clients across Toowoomba North, Oakey and surrounding areas. The role involves conducting intake, assessing needs, and developing personalised care plans while delivering in-home services and transport assistance.

You will travel between client homes, work with the care team to review goals, and provide compassionate, client-centred care that enhances quality of life.

Qualifications

• Certificate III in Aged Care or equivalent qualification required.
• Current Australian Driver's Licence required.
• Vehicle must be roadworthy and insured for work use.

Responsibilities

• Conduct initial client intake and screening assessments and develop client-centred care plans.
• Provide in-home care including personal self-care, light housekeeping, social support, transport and shopping assistance.
• Maintain accurate client information in CRM & CMS; keep case notes up to date.
• Engage in ongoing care discussions, planning, assessment, and reviews with the care team.
