BlueCare in Rockhampton and surrounding areas is seeking a Senior Personal Carer to conduct initial client intake and screening, develop individualized care plans and deliver high-quality in-home support tailored to each client's needs.

You will drive visits across communities, manage client information in CRM/CMS, and work with the care team to review goals and outcomes, while promoting independence and safety.

Qualifications

• Experience with technology incl. smartphones, tablets, laptops
• Current Australian Driver’s Licence (Green Provisional minimum)
• Roadworthy & insured vehicle for work

Responsabilités

• Conduct initial client intake and screening assessments
• Develop and review individualized care plans
• Manage client information in CRM & CMS and keep notes up to date
• Engage in ongoing care discussions, planning, assessments and reviews
• Provide in-home support and assist with daily activities and transportation as needed
