Job summary

BlueCare is seeking a Senior Personal Carer to conduct initial client intake and screening, develop client-centred care plans and deliver quality, tailored in-home care across multiple Brisbane areas. The role requires attention to detail, empathy and strong organisational skills to coordinate with the care team.

You will visit clients’ homes, support daily activities, manage information in CRM/CMS and contribute to ongoing reviews to achieve best outcomes.

Qualifications

• Respite Centre Experience required.
• Current Australian Driver's Licence (Green Provisional minimum).
• Roadworthy & insured vehicle to use for work.

Responsibilities

• Conduct initial screening assessments and develop care plans.
• Manage client information in CRM & CMS; keep case notes up to date.
• Engage in ongoing care discussions, planning, reviews and evaluating goals.
