Job summary

UnitingCare Children, Young People and Families – Rockhampton QLD is seeking a Senior Personal Carer to work with BlueCare in the community. You’ll conduct initial intake, assess needs, and develop personalised care plans with a client-centred approach, delivering in-home support across personal care, cleaning, social support and transport.

Ideal candidates hold Certificate III Aged Care or equivalent, a current Australian driver’s licence, and a roadworthy vehicle.

Qualifications

• Certificate III Aged Care, Individual Support or equivalent qualification.
• Current Australian Driver’s Licence (Green Provisional minimum).
• Roadworthy & insured vehicle to use for work.
• Experience with technology incl. smartphones, tablets, laptops etc.
• Ability to satisfy probity checks required by legislation or policies.

Responsibilities

• Conduct initial intake and screening assessments and develop client-centred care plans.
• Manage client information in CRM & CMS and keep case notes up to date.
• Engage in ongoing care discussions, planning, reviews and goal evaluation with the care team.
• Travel to clients' homes for scheduled visits and assist with daily activities.
• Provide care per each client’s Care Plan and support community activities.

About the role:

As aSenior Personal Carer,you’llconduct initial client intake and screening assessments and developing and reviewing individualised care plans with a client-centred approach while providing quality care based on clients’ needs.

• Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.
• Management of client information in CRM & CMS ensuring all details and case notes are kept up to date
• Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team and as required.
• Spending your time on the road completing scheduled visits, interacting one-on-one with clients within their homes supporting with daily activities
• Providing care in accordance with their individualised Care Plan
• Supporting with community based recreational and social activities
• Transportation and assistance with shopping and appointments.
