Job summary

BlueCare, part of UnitingCare Queensland, is seeking a Senior Personal Carer to conduct client intake, assess needs, and develop client-centered care plans. You will travel to client homes delivering personal care, domestic support, transport and shopping assistance, while updating CRM/CMS records and coordinating with the care team.

Required are Certificate III Aged Care or equivalent, a valid Australian driver’s licence, and a roadworthy vehicle.

Qualifications

• Certificate III Aged Care or equivalent qualification.
• Current Australian Driver's Licence (Green Provisional minimum) and access to a roadworthy vehicle.
• Experience with technology (smartphones, tablets, laptops).
• Ability to complete probity checks as required.

Responsibilities

• Conduct initial client intake and screening assessments and develop care plans.
• Manage client information in CRM & CMS; keep case notes up to date.
• Provide in-home care including personal self-care, domestic support, transport and shopping.
• Engage in ongoing care discussions and reviews with the care team.
• Travel to client homes for scheduled visits and support daily activities.

• Remuneration: $37.77. p/hr + 12% Super + Not for Profit Salary Packaging (Increases your take home pay) + Allowances & Travel Reimbursement (KM's) + Employee discounts

About The Role

As a Senior Personal Carer, you'll conduct initial client intake and screening assessments and developing and reviewing individualised care plans with a client-centered approach while providing quality care based on clients' needs.

• Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.
• Management of client information in CRM & CMS ensuring all details and case notes are
• kept up to date
• Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals
• and outcomes in consultation with the care team and as required.
• Spending your time on the road completing scheduled visits, interacting one-on-one with clients within their homes supporting with daily activities
• Providing care in accordance with their individualised Care Plan
• Supporting with community based recreational and social activities
• Transportation and assistance with shopping and appointments.

Role Requirements

• Certificate III Aged Care, Individual Support or an equivalent qualification - Essential
