Job summary

Gladstone Community Linking Agency (GCLA) is hiring a Senior NDIS Care Coordinator to lead the delivery of supports for participants with complex needs in Gladstone and Rockhampton. You will coordinate SILS funding, develop individual plans, and work with clinical teams to ensure safe, person-centred services aligned with NDIS plan outcomes.

You will oversee intake, oversee plans, and guide frontline staff while maintaining participant choice, dignity and independence.

Qualifications

• Diploma or higher qualification in Disability, Community Services, Allied Health Assistance, Mental Health or another relevant field.
• Demonstrated experience working within the NDIS and supporting people with complex needs.
• Knowledge of SILS and high-intensity daily personal activity supports.
• NDIS Worker Screening Check
• Blue Card / Working with Children Check
• Current Australian Driver Licence

Responsibilities

• Coordinate intake and onboarding of participants with complex support needs or SILS funding.
• Develop, implement and review individual support plans with participants, families and professionals.
• Coordinate supports for high-intensity and complex care requirements.
• Collaborate with nurses, allied health professionals and medical teams.
• Support implementation and monitoring of Positive Behaviour Support Plans.
• Ensure restrictive practices are authorised, documented, monitored and reported.
• Monitor participant risks, incidents and changes in support needs.
• Coordinate internal and external services to align with goals and funding.
• Support participant transitions between services and providers.
• Maintain Schedules of Support and Rosters of Care.
• Coach and support frontline employees delivering complex care.
• Build strong relationships with participants, families, guardians and stakeholders.
• Maintain accurate case notes and participant records.
• Support NDIS compliance, audits, provider reporting and continuous improvement.
