Job summary

The Health Lane Pty Ltd is seeking an experienced Support Worker to assist a participant with complex behaviours. You will provide individualised care, work with the participant's support network, and implement Behaviour Support Plans to promote safety and wellbeing.

You will collaborate with a multidisciplinary team, de‑escalate when needed, and ensure accurate documentation. This role is contractor-based with flexible shifts and opportunities to develop in behaviour support strategies.

Qualifications

• Proven experience supporting people with complex behaviours or high-intensity needs (essential).
• Training or willingness to train in Positive Behaviour Support and restrictive practice awareness.
• Current First Aid & CPR or willingness to obtain.

Responsibilities

• Provide skilled, individualised support to participants with complex behaviours, following Behaviour Support Plans.
• Support daily living, community access and personal goals.
• Maintain accurate incident reports and shift documentation.
• Collaborate with Behaviour Support Practitioners, families and the care team.
• Respond calmly to challenging situations and uphold participant rights.
