Job summary

Optimum Health Solutions is seeking an experienced Behaviour Support Practitioner to join our multidisciplinary team across NSW and Tasmania. You will conduct assessments, develop Behaviour Support Plans, and deliver Positive Behaviour Support interventions in clinic, home, community and school settings.

Ideal candidates have current NDIS registration at Core level or above, strong communication skills, and a passion for improving quality of life for children, adolescents and adults with complex

Qualifications

• Experience as a Behaviour Support Practitioner or in a similar behavioural support role.
• Strong understanding of Positive Behaviour Support frameworks.
• Experience conducting assessments and developing Behaviour Support Plans.
• Current NDIS Behaviour Support Practitioner registration (Core level or above).

Responsibilities

• Conduct behavioural support assessments.
• Develop high-quality Behaviour Support Plans.
• Deliver evidence-based Positive Behaviour Support interventions.
• Provide support in clinic, home, community and school environments.
• Work with children, adolescents and adults with complex support needs.
• Collaborate with families, carers, support coordinators and other stakeholders.
• Contribute to meaningful outcomes that improve quality of life and community participation.
