Job summary

Regis Home Care is seeking a Client Services Manager to lead care planning for older Australians in the western suburbs (Werribee, Hoppers Crossing, Tarneit and surrounds). You will develop individualised care plans, assess client needs, and coordinate with clinicians and Home Support Workers to support independence at home.

The role emphasizes compassion, partnership with families, and ability to navigate Home Care Package funding requirements. A company car is provided for client visits.

Qualifications

• Diploma level qualifications in health/ aged care/ disability/ social welfare (preferred).
• Strong interpersonal and networking skills to connect with local service providers and referrers.
• Ability to apply person-centred care principles in planning and delivering services.

Responsibilities

• Develop care and support plans aligned with client needs and aspirations.
• Conduct assessments and create person-centred care plans focusing on independence and wellbeing.
• Assist families to understand Home Care Package funding and guidelines.
• Collaborate with clinical experts, allied health professionals and Home Support Workers to meet client needs.
• Travel to visit clients; a company car will be provided when required.
