Regis Home Care is seeking a compassionate Client Services Manager based in Blackburn South to cover southeast Melbourne, Bayside and Mornington Peninsula. You will lead care planning, assessments and client advocacy to enable independent living at home with dignity and safety.

As part of the home care team, you will coordinate with clinical experts and local providers, ensuring personalised supports align with client goals and funding guidelines.

Formación

• Diploma level qualifications in health/ aged care/ disability/ social welfare (preferred).
• Attention to detail and a practical attitude.
• Experience in health or aged care sector preferred.
• Strong networking with local service providers and referrers.

Responsabilidades

• Develop care and support plans aligned with client needs and aspirations.
• Conduct assessments to inform person-centred care plans.
• Assist clients and families to use Home Care Package funding per guidelines.
• Build partnerships with clinical experts, allied health professionals and Home Support Workers.
