Job summary

Court Services Victoria in Warrnambool is seeking an experienced Advanced Case Manager to deliver high-quality, person-centred support to participants in the Court Integrated Services Program (CISP). You will provide advanced, holistic case management to individuals with complex needs, collaborating with clients and stakeholders to tailor plans that promote recovery and reduce risk.

Qualifications

Tertiary qualification (minimum Diploma level; Degree highly desirable) relevant to case management with forensic clients.
Demonstrated experience or capability in case management of clients with complex needs, knowledge in at least one area.
Detailed knowledge of mental health or disability service systems and stakeholder relationships.
Knowledge of evidence-informed practice models for working with clients with complex needs.
Excellent case formulation and written communication skills.

Responsibilities

Conduct assessments and develop person-centred support and treatment plans.
Identify and manage risk to clients and others.
Maintain accurate, compliant documentation.
