Job summary

Court Services Victoria is seeking an experienced Advanced Case Manager to support participants in the Court Integrated Services Program (CISP). You will deliver advanced, holistic case management for individuals with complex needs, including mental illness, homelessness, and substance use issues, across the Warrnambool Barwon South Western region.

Qualifications

• Minimum tertiary qualification (Diploma level; Degree desirable) in fields such as social work, psychology, welfare, AOD, or related field.
• Experience or capability in case management of clients with complex needs.
• Knowledge of mental health, disability, or AOD systems and stakeholder relationships.

Responsibilities

• Deliver advanced case management using a strengths-based, evidence-informed approach.
• Build strong relationships with clients and stakeholders.
• Conduct assessments and develop person-centred support and treatment plans.
• Identify and manage risk to clients and others.
• Maintain accurate, compliant documentation.
• Contribute to service quality and continuous improvement.
