Opal HealthCare’s Surrey Hills Gardens Care Community in Surrey Hills, near Melbourne, seeks a Care Manager to lead clinical care and a high-performing team. You will partner with the GM and regional teams to deliver safe, person-centred care under Aged Care Act 2024 and AN-ACC standards.

The role balances governance, workforce capability, and operational performance, within a supportive, scale-enabled national provider.

Qualifications

RN with AHPRA registration (minimum 3 years)
Experience leading clinical teams in residential aged care
Strong understanding of Aged Care Quality Standards and AN-ACC
Ability to lead change, manage risk and improve outcomes
Values-led leader with mentoring and team-building skills

Responsabilités

Lead clinical care delivery ensuring compliance with Aged Care Standards and regulations
Oversee care planning, admissions reviews, and changes in resident condition
Monitor and analyse clinical indicators, incidents and risk trends
Apply evidence-based practice to improve outcomes
Build relationships with GPs, hospitals and allied health providers
Ensure adherence to Aged Care Act 2024 requirements and reporting obligations
Drive continuous improvement, audit readiness, and clinical excellence
Lead compliance with incident management and accreditation standards
Foster a values-led, high-performing culture
Oversee education, onboarding and competency development
