Opal HealthCare is seeking a Care Manager to join the Mulgrave Rise Care Community leadership team. This hands-on role partners with the General Manager to deliver safe, high-quality, person-centred care and to lead high-performing clinical teams.

Qualifications

• RN with AHPRA registration and minimum 3 years in residential aged care.
• Strong understanding of Aged Care Quality Standards and AN-ACC.
• Ability to lead change, manage risk and improve outcomes.
• Visible, values-led leader with mentoring and team-building skills.

Responsibilities

• Lead clinical care delivery ensuring compliance with Aged Care Quality Standards.
• Oversee care planning, admissions reviews and resident condition changes.
• Monitor clinical indicators, incidents and risk trends.
• Apply evidence-based practice to improve outcomes.
• Build relationships with GPs, hospitals and allied health providers.
• Ensure adherence to Aged Care Act 2024 requirements and reporting obligations.
• Drive continuous improvement, audit readiness and clinical excellence.
• Lead compliance with incident management and accreditation standards.
• Foster a values-led, high-performing culture.
• Oversee education, onboarding and competency development.
