Job summary

NSW Health is seeking a Registered Nurse to join the Western Sydney Local Health District at Cumberland Hospital. The role supports older people to regain independence after hospital stays, within a multidisciplinary team and a trusted NSW Health framework.

You will deliver person-centred care, coordinate recovery goals with families and health professionals, and maintain high clinical standards while contributing to service excellence.

Qualifications

• Bachelor of Nursing or higher.
• Registration: AHPRA NMRB of Australia Registration.
• License: NSW Drivers Licence.
• Demonstrated recent knowledge & nursing experience in aged care, rehabilitation & community.

Responsibilities

• Deliver safe, compassionate and high-quality nursing care for older people after hospitalisation.
• Develop and achieve individualised care goals with clients, families and health professionals.
• Assess and coordinate care: plan, implement and evaluate holistic nursing care tailored to clinical needs.
• Empower clients and families: communicate clearly to support informed decisions and recovery.
• Collaborate with internal and external health professionals to ensure continuity of care.
• Maintain clinical documentation and uphold professional and aged care standards.
• Assist with screening referrals and onboarding eligible clients to the programme.
• Balance priorities and resources while delivering person-centred care.
