Qualifications

• Current registration as a Registered Nurse with the Nursing & Midwifery Board of Australia.
• Minimum 3 years’ experience as a Registered Nurse in aged care, rehabilitation or related area.
• Post graduate qualification or interest in this field.

Responsibilities

• Plan, coordinate and deliver high-quality nursing and case management for Transitional Aged Care patients.
• Commit to quality improvement and evidence-based practice within the TAC team.
• Collaborate with multidisciplinary colleagues to ensure safe, effective care.
