McLean Care, Inverell, NSW, is seeking an experienced Registered Nurse to step into aClinical Care Coordinator role. You will lead the care team to coordinate high-quality, person-centred clinical care while supporting residents and families.

Reporting to the General Manager, you will ensure individual care plans reflect clinical needs and goals, while maintaining documentation and governance standards in residential aged care.

Qualifikationen

• Bachelor of Nursing and current AHPRA RN registration.
• Experience in residential aged care and clinical leadership.
• Strong clinical assessment, documentation and nursing practice skills.
• Experience in multidisciplinary teams and excellent communication.

Aufgaben

• Coordinate and review person-centred resident care plans.
• Complete clinical assessments and respond to changes in residents' needs.
• Support admissions and ensure appropriate assessments.
• Collaborate with residents, families and healthcare professionals.
• Maintain high standards of clinical documentation.
• Provide and support direct nursing care when required.
• Monitor wounds, falls, and clinical deterioration.
• Support medication management, infection control and audits.
• Assist with investigation and close-out of clinical incidents.
• Guide, support and develop nurses and care staff; support orientation.
