E4 People is partnering with a well-established residential aged care provider in Nambucca Heads, NSW, to appoint a Clinical Care Coordinator. This full-time role leads a care team, coordinates admissions, and develops person-centred care plans to deliver safe, high-quality clinical care.

The successful candidate will be a Registered Nurse with 2+ years' aged care experience, a passion for clinical excellence, and the ability to mentor others within a multidisciplinary team.

Qualifications

• Bachelor of Nursing with current AHPRA registration.
• Minimum 2 years' nursing experience in residential aged care.
• Experience in a similar clinical care role highly regarded.
• Strong clinical assessment, documentation and evidence-based practice.
• Collaborative multi-disciplinary team involvement.
• Excellent communication and interpersonal skills.
• Confidence engaging with residents, families and professionals.
• Proficient in Microsoft applications and clinical software systems.
• Understanding of aged care accreditation processes.
• Ability to meet Aged Care Worker Screening requirements.

Responsabilités

• Coordinate admission of new residents with clinical assessments and care plans.
• Lead, mentor and support the care team to deliver high-quality care.
• Provide hands-on nursing care including medication administration and wound management.
• Maintain accurate clinical documentation.
• Oversee infection prevention and control practices.
• Ensure compliance with regulatory standards and accreditation.
• Communicate effectively with residents, families and providers.
• Collaborate with multidisciplinary teams for positive resident outcomes.
• Contribute to quality improvement and clinical excellence initiatives.
• Support a teamwork-focused, person-centred care culture.
