Job summary

Standard Nursing Care Pty Ltd in Sydney is seeking a full-time NDIS care professional to provide person-centred support according to individual care plans and goals. You will assist with activities of daily living, develop independent living skills and promote wellbeing.

The role requires certificates in support services, at least 1 year experience in disability or related fields, strong communication and problem-solving abilities, and compliance with NDIS and WHS standards.

Qualifications

• Certificate III or Certificate IV in Individual Support, Disability, Community Services, Aged Care, or a related qualification.
• Minimum 1 year experience in disability services, residential care, community care or supported accommodation.
• Demonstrated understanding of person-centred care principles and NDIS support delivery.
• Strong communication, interpersonal and problem-solving skills.
• Current First Aid and CPR Certificate.
• NDIS Worker Screening Check and Working with Children Check where required.

Responsibilities

Sourcify Pty Ltd Key Responsibilities Provide person-centred support to participants in accordance with individual care plans and NDIS goals. Assist residents with activities of daily living, including personal care, dressing, grooming, hygiene and mobility support. Support participants to develop independent living skills such as meal preparation, budgeting, household management and self-care.

Facilitate community access, social participation, recreational activities and appointments. Monitor participant wellbeing and promptly report changes in health, behaviour or support needs. Administer or assist with medication in accordance with organisational policies and relevant legislation.

Maintain accurate case notes, progress records, incident reports and documentation. Implement positive behaviour support strategies and promote a safe, respectful living environment. Assist with household duties including cleaning, laundry, shopping and meal preparation.

Support participants with transport arrangements and access to community services. Work collaboratively with families, allied health professionals, support coordinators and other stakeholders. Ensure compliance with NDIS Practice Standards, Work Health and Safety (WHS) requirements and organisational policies.

Promote participant choice, dignity, cultural respect and human rights at all times. Requirements Certificate III or Certificate IV in Individual Support, Disability, Community Services, Aged Care, or a related qualification. Minimum 1 year experience in disability services, residential care, community care or supported accommodation.

Demonstrated understanding of person-centred care principles and NDIS support delivery. Strong communication, interpersonal and problem-solving skills. Ability to work independently and as part of a multidisciplinary team.

Experience supporting people with complex needs, disabilities or challenging behaviours will be highly regarded. Current First Aid and CPR Certificate.

NDIS Worker Screening Check (or ability to obtain). Working with Children Check (where required).
