Job summary

Hometown Nursing Care in Sydney is seeking compassionate Disability Support Workers to join our growing NDIS team. You will provide person-centred support to participants to help them live independently, participate in their community and achieve their individual goals.

Key responsibilities include personal care, assisting with daily living and community access, plus maintaining accurate notes and documentation. The role emphasizes respect, reliability and participant empowerment.

Qualifications

• Certificate III/IV in Individual Support or related qualification preferred.
• NDIS experience is highly regarded.
• NDIS Worker Screening Check and First Aid/CPR required.
• WWCC where required; valid driver's licence preferred.

Responsibilities

• Provide personal care and assistance with daily living.
• Support participants with community access and social activities.
• Assist with appointments, shopping and everyday tasks.
• Encourage independence, choice and confidence.
• Follow individual NDIS support plans and maintain progress notes.
• Work respectfully with participants, families and other professionals.

• Current driver licence and reliable vehicle preferred
