Imagine Independence is recruiting a Resettlement and Recovery Support Worker based in Liverpool to support men on the offender pathway with complex needs, including personality disorders, through housing and community-based recovery services.

You will build therapeutic relationships, develop person-centred support plans, and work with housing partners to secure accommodation while promoting wellbeing and social inclusion. Shift pattern involves 12-hour shifts on a rolling rota.

Formación

• Experience supporting individuals with complex mental health needs.
• Knowledge of securing accommodation and tenancy sustainment.
• Commitment to reducing re-offending and improving wellbeing.

Responsabilidades

• Develop therapeutic relationships to support individuals in emotional states.
• Develop and review person-centered plans with care partners involved.
• Encourage access to employment, education, volunteering and community activities.
• Work with housing partners to secure ongoing accommodation.
• Deliver transition support to independent living.
• Undertake risk assessments and create risk management plans.
• Maintain records for evaluation purposes.
