Job summary

Chosen Family in Adamstown, NSW is seeking male-identifying Support Workers for a community access and reintegration project supporting a 29-year-old participant with schizophrenia and cerebral palsy as he transitions back into the community after custody.

These casual, part-time roles involve building routines, accompanying to appointments, and facilitating access to local services and activities, with up to 17.5 hours per week. Commencement anticipated December 2026, subject to confirmation.

Qualifications

• Experience supporting people with psychosocial disabilities or complex support needs.
• Understanding of schizophrenia and recovery‑oriented practice.
• Experience supporting people through transitions, including community reintegration or justice‑related support.
• Trauma‑informed approach emphasizing dignity, choice and safety.

Responsibilities

• Establish practical daily routines after transition into community living.
• Encourage participation in meaningful activities and community programs.
• Provide consistent, respectful support while allowing client to make choices.
• Build confidence with local services and community resources.
• Accompany to appointments, errands, and community activities.
• Follow support plans and maintain documentation.
