Qualifications

• Minimum 2 years of Level 3 Support Coordination experience.
• Ability to work autonomously from a home-based setting.
• Solid grasp of the NDIS Framework and relevant community resources.
• Excellent verbal and written communication skills.
• Motivation to support individuals with diverse needs (mental health, physical, cognitive).
• Sensitivity and respect for diverse cultures.

Responsibilities

• Assessment & Planning: Conduct thorough assessments and develop effective support plans.
• Service Navigation: Connect participants with appropriate and beneficial service providers.

• Documentation & Reporting: Maintain accurate records and provide timely updates.
• Crisis Support: Offer crucial assistance during challenging times.
