Job summary

Lives Lived Well in Bundaberg is seeking a Program Coordinator to support the Tokoko Integrated Recovery Pathways Program in the Wide Bay region. The role focuses on transition planning, housing pathways and ongoing recovery support to help clients move toward independent living.

The position is based in Bundaberg, offers a fixed-term arrangement to 30 June 2027, with regular travel across Bundaberg, Hervey Bay and the wider Wide Bay area.

Qualifications

• Qualifications in Community Services, Human Services, Social Work, Psychology, Counselling or related discipline, and/or relevant experience.
• Experience supporting people through complex pathways, transitions or recovery-focused programs.
• Experience in case management, housing support, tenancy support or community services.
• Understanding of recovery-oriented, trauma-informed and person-centred practice.
• Current Australian Driver Licence.

Responsibilities

• Support clients through pre-admission, transition and post-residential recovery pathways.
• Coordinate access to transitional accommodation and support successful tenancy outcomes.
• Assist clients to build independent living skills, recovery supports and community connections.
• Develop and implement transition and recovery plans tailored to individual needs.
• Support clients to access housing, health, employment, education and community services.
• Strengthen continuity of care through effective planning, communication and service coordination.
• Build and maintain strong referral pathways and relationships with community stakeholders.
• Support relapse prevention and ongoing recovery planning.
• Advocate for clients and assist them to navigate complex service systems.
• Maintain accurate documentation, case notes and reporting requirements.
