Job summary

ForHealth is seeking passionate Registered Nurses or Enrolled Nurses to join our team as a Chronic Disease Management Health & Wellbeing Nurse. You will work with GPs and allied health professionals to support preventative health, education, care coordination and ongoing patient support.

In a collaborative environment, you’ll help patients manage chronic conditions, coordinate care, and promote health coaching while ensuring Medicare compliance and accurate records.

Qualifications

• Current AHPRA registration as a RN or EN.
• Experience in Chronic Disease Management, Care Plans and Health Assessments preferred.
• General Practice/Primary Healthcare experience preferred.
• Excellent communication and interpersonal skills.
• Strong organisational and time management abilities.
• Proactive, patient-focused approach to care.

Responsibilities

• Support patients to manage chronic conditions and achieve positive health outcomes.
• Provide health coaching and promote positive behaviour change initiatives.
• Coordinate care with GPs, specialists, allied health providers and community services.
• Conduct and coordinate clinical services including ECGs, Spirometry, ABI testing and DMMRs where appropriate.
• Maintain accurate patient records and ensure Medicare compliance requirements are met.
• Proactively identify eligible patients for chronic disease management and preventative health services.
• Build strong relationships with patients and provide ongoing education and support.

About You

To be successful in this role, you will have:

• Current AHPRA registration as a Registered Nurse (RN) or Enrolled Nurse (EN)
• Previous experience in Chronic Disease Management, Care Plans and Health Assessments (highly regarded)
• Prior experience in General Practice or Primary Healthcare (preferred)
• Strong understanding of preventative health, health promotion and chronic disease management principlesExcellent communication and interpersonal skills
• Strong organisational and time management abilities
• The ability to motivate and engage patients in improving their health outcomes
• A proactive, patient-focused approach to care
