ForHealth is seeking a Registered Nurse or Enrolled Nurse to join Maroubra Medical Centre and lead Chronic Condition Management, working with GPs and patients to improve health outcomes through preventative care.

Qualifications

• Current AHPRA registration as a Registered Nurse or Enrolled Nurse.
• Experience in Chronic Condition Management is highly regarded.
• Excellent communication and interpersonal skills are essential.

• Proactive, patient‑focused approach to care is valued.
• Ability to work independently and as part of a team.

Responsibilities

• Prepare and coordinate Chronic Condition Management Plans and Reviews.
• Support patients in managing chronic health conditions and achieving positive outcomes.
• Conduct health assessments and preventative health activities.
• Work closely with GPs and the wider multidisciplinary healthcare team.
• Provide patient education and ongoing support.
• Maintain accurate clinical records and appointment administration.
