Job summary

ForHealth Group is seeking a Registered Nurse or Enrolled Nurse to join Kedron Medical Centre in a part-time capacity. The role focuses on Chronic Disease Management, working with GPs and patients to improve outcomes through preventative care and ongoing support.

You will participate in gpmp/tca activities, health assessments and patient education, while using tools like POLAR, PenCAT and HotDocs to identify eligible patients. A collaborative, patient-centred team environment is provided.

Qualifications

• Requires current AHPRA registration as a Registered Nurse or Enrolled Nurse.
• Experience in Chronic Disease Management highly regarded.
• Experience in General Practice or Primary Healthcare preferred.

Responsibilities

• Prepare and coordinate GP Management Plans (GPMPs) and Team Care Arrangements (TCAs).
• Support patients in managing chronic conditions and achieving positive health outcomes.
• Conduct health assessments, including preventative and chronic disease screening.
• Collaborate with GPs, specialists and the broader healthcare team.
• Provide patient education, coaching and ongoing support to encourage healthy behaviours.
• Coordinate referrals to allied health professionals and external providers.
• Perform clinical services including ECGs, spirometry, ABI testing and urinalysis where appropriate.
• Utilise POLAR, PenCAT and HotDocs to identify eligible patients.
• Maintain accurate clinical records and appointment administration.
• Support preventative health initiatives and outreach programs.

About You
