Job summary

Right at Home Darling Downs is seeking a Registered Nurse or Enrolled Nurse to join as Clinical Care Partner / Care Coordinator in Toowoomba. This role focuses on delivering high-quality, person-centred care and coordinating services across the region.

You’ll work with the Managing Directors and Care Manager to ensure clinical standards are met and exceeded, with a focus on meaningful care planning and client satisfaction.

Qualifications

• AHPRA registration as Registered Nurse or Enrolled Nurse is essential.
• Experience developing care plans and coordinating client services.
• Knowledge of Aged Care Quality Standards.
• Current CPR & First Aid certification.
• Police Check and Working with Children Check current.
• Valid Driver’s Licence and reliable vehicle.

Responsibilities

• Conduct client assessments and develop individualised, meaningful care plans.
• Oversee clinical care, ensuring care documentation is accurate and compliant.
• Provide mentorship, supervision and clinical guidance to Caregivers.
• Liaise with clients, families and health professionals to ensure continuity of care.
• Conduct in-home clinical visits and assessments.
• Identify opportunities to improve care processes and outcomes.
• Participate in an on-call rotation for after-hours support.

Your Role

As a Clinical Care Partner / Care Coordinator, you’ll combine your clinical expertise with leadership and coordination skills to ensure clients receive high-quality, person-centred care.

You’ll work closely with our Managing Directors and Care Manager to deliver exceptional support and ensure our clinical standards are met and exceeded.

Key Responsibilities

Conduct client assessments and develop individualised, meaningful care plans.

Oversee clinical care, ensuring all care documentation is accurate and compliant.

Provide mentorship, supervision and clinical guidance to Caregivers.

Liaise with clients, families and health professionals to ensure continuity of care.

Conduct in-home clinical visits and assessments.

Identify opportunities to improve care processes and outcomes.

Participate in an on-call rotation for after-hours support (shared roster).

About You

You are a Registered Nurse or Enrolled Nurse who is passionate about making a difference and thrives in a collaborative, community-focused environment.

Current AHPRA registration as a Registered Nurse or Enrolled Nurse.

Experience developing care plans and coordinating client services.

Knowledge of Aged Care Quality Standards.

Current CPR & First Aid, National Police Check and Working with Children Check.

Current Driver’s Licence and reliable vehicle.

Reside in or near the Toowoomba Region.

Strong communication and interpersonal skills, with a genuine passion for person-centred care.
